Act 4 of Path of Exile 2 contains several side activities that are easy to overlook while moving through the campaign. Tribal Medicine is one of them, and its unusual progression can make the objective seem more complicated than it really is. Your POE2 Currency and equipment are useful for handling the area's challenges, but knowing when to deliver the Shark Fin is the most important part of completing this quest efficiently.
Begin With the Great White One
The Tribal Medicine side quest centers on a Shark Fin obtained after defeating the Great White One on Whakapanu Island. Once the boss has been defeated, the quickest route is to return to Kingsmarch by using a portal.
At Kingsmarch, find Makoru near the boat. Speak with him and choose the option to travel to Ngakanu, the southern island. During the earlier portion of Act 4, this location remains accessible and provides the easiest opportunity to finish the quest.
After arriving at Ngakanu, enter the Karui village and check the right side of the area. Kaimana should be waiting there. Speak to her while the Shark Fin is in your possession, and she will accept the item, allowing you to progress the side quest.
Complete the Quest Before the Story Changes
Turning in the Shark Fin soon after defeating the Great White One is the most convenient approach. It avoids unnecessary delays and prevents you from carrying a quest item through several additional campaign areas.
Act 4 changes as the main storyline develops, so completing available side objectives before major story events can make your progression easier to manage. If Kaimana is accessible, there is little reason to postpone the turn-in.
This early route is especially useful for players who prefer to clear side content as they encounter it. After finishing the quest, you can continue with the main storyline without needing to keep track of the Shark Fin.
Understand the Ngakanu Corruption Phase
The quest becomes confusing if you progress beyond a particular point in the main campaign. After defeating Benedict's first herald, Ngakanu becomes corrupted and is temporarily unavailable.
During this phase, Kaimana cannot be found at her earlier location. Returning to the village will not allow you to complete Tribal Medicine because the area itself has changed as part of the story.
This does not permanently prevent completion. The quest remains available, so there is no reason to restart your campaign or worry that the Shark Fin has become useless.
Keep the Shark Fin Safe
If Ngakanu has already become corrupted, simply keep the Shark Fin until you reach the end of Act 4. Leaving it in your inventory is the most straightforward solution.
It is important not to discard the item just because Kaimana is temporarily unavailable. The quest does not require you to obtain another Shark Fin later, and abandoning the item can create unnecessary complications.
Instead, continue working through the main storyline. Once the campaign reaches its conclusion, Kaimana becomes available again in a different location.
Locate Kaimana After Completing Act 4
Players who miss the early turn-in have a reliable alternative after finishing the Act 4 campaign. Kaimana returns permanently to Kingsmarch, so there is no need to revisit the corrupted version of Ngakanu.
Travel to Kingsmarch after completing the main story and look around the Karui catamaran area. Kaimana can be found near the ship that is docked across the pier from the main waypoint boat.
Speak with her while carrying the Shark Fin to finish the quest. This late-game route is particularly helpful for players who prefer completing the main campaign first and cleaning up side quests afterward.
Pick the Right Permanent Lesson
Tribal Medicine is worth completing because its reward has become considerably more valuable in newer versions of the game. Changes introduced around Patch 0.5 replaced the earlier reward structure with a choice between two permanent lessons.
Kaom's Lesson provides defensive benefits associated with Armour, Evasion, and Energy Shield. Characters that rely on several defensive layers may find this option particularly useful.
Rakiata's Lesson takes a different approach to defense. Its effects can interact with mechanics such as Armour, elemental mitigation, and shield recovery. The best selection depends on your character's build and the defensive systems you already use.
Because the chosen lesson provides a permanent character benefit, consider your current setup before confirming your selection. A reward that complements your existing defenses can remain useful throughout future progression.
Finish Tribal Medicine Without Extra Backtracking
The simplest strategy is to defeat the Great White One, return to Kingsmarch, sail to Ngakanu, and speak with Kaimana in the Karui village before advancing too far through Act 4. If the corruption phase has already begun, keep the Shark Fin and continue the campaign instead of trying to force an early turn-in.
After completing the Act 4 story, return to Kingsmarch and visit Kaimana near the Karui catamaran. Deliver the Shark Fin and choose the permanent lesson that best suits your build. With the quest completed, you can move forward with your character development and focus on valuable endgame goals, including obtaining a POE 2 Mirror of Kalandra when the time is right.
